Postpartum Visits (HEDIS)

Onsite to perform HEDIS measure postpartum visits by a licensed NP professional within 21-56 days after delivery
Visit to include post partum assessment of mother for both clinical and psychosocial aspects
Assessments to be reported to HMO’s/IPA’s, etc via an online secure portal
Coordinate followup appointments
NP to report any important or urgent postpartum findings to Healthcare organizations immediately if necessary
Secure portal access for Pt charting/status

Bone Density Scan (DEXA SCAN)

Onsite performs HEDIS measure DEXA Scan on female patients qualifying for measure with recent fractures. The in-home procedure is performed by a licensed and insured LVN via radial ultra-sound scan. Reports are directly forwarded to PCP or can be accessible via our secure web portal.

Spirometry (Pulmonary Function)

Measuring and assessing lung function is an essential tool in the diagnosis and treatment of many respiratory diseases, such as asthma, COPD, and Pulmonary Fibrosis. Onsite leverages its field expertise to reach those clients in their homes and deliver this valuable service. In doing so, we have become a reliable ally in assisting healthcare providers reach their quality of care goals and objectives.

Anti-Coagulation (PT/INR Testing & Reporting)

For better compliance, improved outcomes, and minimal adverse events, our in-home anti-coagulation monitoring services for Coumadin (Warfarin) patients have proven to be of great value to our clients. Often, these tests are performed via a simple Fingerstick with results reported at bedside. Reporting is also available via our secured web portal.
